Your child must be 5 years old before December 1, 2010 to enroll them for the 2010-2011 school year. Your child must complete a Hearing & Vision Screening before starting School.

Registration materials:
- May be picked up at the Transportation Department office weekdays from 7:30 a.m. until 4:30 p.m.
- Can be mailed to you by calling 734-654-3433.

Residency Affadavit - to be completed ONLY if parent is not the owner of the home. Submit completed, notarized and with 2 of the homeowners utility bills.
Items Needed to Complete Registration
Registration must be completed by a parent of the student. If not the parent, appropriate guardianship papers must be presented.- Parent Drivers License or ID: For photo identification only
- Student's Original Birth Certificate: must have state seal
- Student's Immunization Record
- Proof of Residency: Two utility bills. If utility bills are not available we will accept Mortgage Info or Rental/Lease agreements.
- Hearing & Vision Screening Proof: If the child has not been screened for hearing and vision, please make an appointment with your doctor or the Monroe County Health Department.
If you are missing any of the above information, you can still begin the registration process by bringing in what you do have. The earlier you come in, the better!
